We obtain information about you when you fill in your medical history form, and in the surgery when you are asked about any updates to this. The personal information collected includes:
The information is used to enable the provision of safe dental treatment within the practice. All information is stored securely at the practice and backed up to a remote, secure server daily. This information is not accessible to the public.
If a card payment is taken at the practice, a copy of this payment is held securely on site, this does not show any personal information.
If any lab work is undertaken information is shared between the practice and the laboratory. A confidentiality agreement has been signed between Saltburn Dental Practice and all laboratories used. All information is kept securely by both parties.
When you complete a course of NHS dental treatment some of your information is submitted to the dental practice board, you agree to this when you sign your NHS agreement form which is required as part of your dentist’s contract term.
NHS England set out guidelines on how long information is stored. We adhere to the guidelines and dispose securely and safely any information when it is no longer required.
You have the right to request removal of any data we hold. If you feel your information is not being stored correctly you have the right to contact the ICO for further guidance on this matter.
